1. Consent for Participation: I hereby consent to my child’s participation in the "It Takes A Village Bootcamp," a virtual fitness and wellness program.

2. Assumption of Risk: I understand that participation in this program involves physical activity and I assume all risks associated with my child’s participation.

3. Medical Clearance: I confirm that my child has been cleared by a physician to participate in physical activities.

4. Waiver of Liability: I agree to the terms of the Waiver and Release of Liability and understand that The Power Coach Martize will not be held responsible for any injuries or damages arising from my child’s participation.

5. Media Release: I consent to the recording of sessions and grant permission for my child’s image to be used in promotional materials.
